MXR 6-Band EQ HELP NEEDED

Does anyone have one of these? Im talking the old school Blue boxed one, not the new black one. My EQ board decided it no longer wanted to be attached to the wires that connected it to the jacks and battery, and all wires broke off at the PCB.

If anyone has one of these would you be so kind and take a good closeup photo of how the jacks and battery are wired to the board? I have the schematic but a few of the values are not exactly what I have on the board. So following the schematic only isn't actually helping. My problem lies in that I have 5 solder points to hook up the jacks/battery to the board with only 4 wires. I am almost certain that I am not missing a wire.

Anywho, if someone would take a picture of their wire to board on the pedal I would trade you a scan of the schematic for their trouble as you cannot get this schematic online without paying $7 for it, and Jim Dunlop no longer will sell you one.
