You said the problem went away for a little while and then returned. I know this may sound simplified, but is there any particular time of day/night that something is turned on or off in your house i.e. other electronics. Also, have you tried different outlets to see if there is a difference in the noise? I've seen bad house grounds make amps do weird things. Also, voltages vary from time to time, with people cooking in the afternoon to you name it during the day. Try taking the amp to a friends house and see if it does it there also.
I know what your going thru is hell, I went thru an ordeal for a month, doing all the norms for trouble shooting with no success, and just out of luck I found the PTP board was bad itself, I couldn't beleive it. But that's my kinda luck! I'm the only one in the universe who could possibly get a defective board. Think that won't play havoc on a rooky amp builder! I don't remember if you've been asked this, but have you checked for DC voltage at all your pot wipers?
