Burn spot on NOS Mullard

The 12ax7 mullard I put in my 1959ri (V-3 PI) just got a small burn mark on the top right next to the tip.It was a NIB NOS. My amp has been getting this "ghost " distortion on the low E string.I cleaned my amp but it wont go way, driving me nuts.
